This excellent resource helps to promote effective ministries to American-Born Chinese and there is much useful information for Canadian readers. Included in the zipped file are many years of archived FACE (Fellowship of American Chinese Evangelicals) magazines – another excellent resource. For Children and Their Parents

Child's Registration Form:  Here is the form to be filled out by the parents regarding the child attending our programs.  There must be one form per child.

Parental Information Form:  Here is the form to be filled out by both parents of the child(ren) attending our program.  Only one set of parental forms is required per family.  If parents do not attend our church and/or are not the main person(s) bringing the child(ren) to church, a Guardian Informaton Form needs to be filled out.

Guardian Information Form:  Here is the form to be filled out by the person who brings the child(ren) to church who is NOT the parent.  Only one guardian required per family.  Please note that there is some parental information requested on this form.

Parents: Benny & Ada Wang, church attendees

Children: Derek Wang, Charlotte Wang, Christopher Wang

Forms required: 2 parental forms (one per parent), 3 children forms (one per child)

Example #2

Parents: Matthew and Teresa Fan, not church attendees

Guardian:  Daisy Yau

Children:  Alex Fan, Amy Fan, Sharon Fan

Forms required: 1 guardian form, 3 children forms

Forms optional: 1 or 2 parental forms.
